Please print legibly and answer all questions. If you do not know an answer, consult with your driller. Your driller should be able to provide coordinates and well information.
You must include a map that clearly indicates the area you plan to irrigate. Label what you plan to water (e.g., rice, hay, trees, waterfowl, pond). Use a highlighter and/or pen to label these ares. Google Earth provides a free program that can help with coordinates and maps. Permit applications must be notarized.

Will the proposed well be capable of pumping more than 35 gallons per minute? If yes, a permit is required. Choose the appropriate permit class from the list below:
Class A wells are capable of pumping 200 gpm or less.
Submit a Well Registration form along with a Class A Permit application. The permit application fee is $25.
Class B wells are capable of pumping between 200 and 600 gpm.
Submit a Well Registration form along with a Class B Permit application. Class B Permit applications require board approval. The permit application fee is $50.
Class C wells are capable of pumping more than 600 gpm.
Submit a Well Registration form along with a Class C Permit application. Class C Permit applications require a public hearing. You are not required to attend the hearing. The permit application fee is $75.
Management Zone 2 (MZ2)
Wells located on less than 5 acres:

Rule 3.4.6 - If an applicant requests more than 325 acre-feet of water annually, a hydrogeological report, mitigation plan, water conservation plan, and drought contengency plan are required.
Rule 3.47 b) - A well or well system with an approved operating permit in MZ2 shall not be operated such that the total annual production exceeds one acre-foot per acre owned or operated, or for which a person can show ownership or possession of groundwater rights.
